A. Caloric and macronutrient targets are important

a) You should aim for 800-1,200 calories per day depending on starting BMI.

b) You should also aim for 60-100 grams of protein per day to preserve muscle mass.

c) Under 50 grams of carbohydrate per day is the driver of liver glycogen depletion.

d) You should have fat and no added sugars.

e) You should drink 64+ ounces of fluid per day including water, sugar-free drinks and broth.

B. What you eat is also important

The pre bariatric surgery diet has a three-phase structure.

a) Days 1-7 are the modified diet phase:

You should have 1 protein shake for breakfast 1 cup berries or melon for snack 1 protein shake for lunch 1 protein shake or 6 oz light Greek yogurt for snack and 3-6 oz lean protein + 1 cup cooked -starchy vegetables or 2 cups salad for dinner.

b) Days 8-13 are the liquid- phase if required:

You should have 4-5 protein shakes per day spaced every 3-4 hours.

You should have sugar- clear liquids between shakes, such as water, broth or sugar-free Crystal Light.

You should not have food.

c) Day 14 is the day before surgery:

You should have liquids only until the cut-off time your surgeon provides.

You should have -surgery clear carbohydrate drinks, such as Ensure Pre-Surgery Clear, if approved by your surgeon.

You should have bowel prep with magnesium citrate at noon if required.

C. What’s allowed on the pre surgery diet 

1. Protein shakes, such as whey isolate, plant-based or pre-made low-sugar varieties.

2. Lean protein, such as chicken breast, turkey, fish, lean beef and eggs.

3. Non-starchy vegetables, such as broccoli, spinach, cauliflower, cucumber, peppers and leafy greens.

4. Sugar-free gelatin, popsicles and broth.

5. Water, tea and sugar-free flavoured water.

D. What’s strictly avoided on the pre surgery diet 

1. Bread, rice, pasta, potatoes, corn and peas.

2. Sugar, candy, desserts and sweetened beverages.

3. Fruit. Regular soda.

4. Alcohol of any kind.

5. Fried. Full-fat dairy.

6. Anything not on your surgeons approved list.

The 3 Month Pre Bariatric Surgery Diet- When It’s Required

The 3 month pre bariatric surgery diet is required by some insurance companies not by necessity. Many insurers mandate documented physician-supervised weight loss for 3-6 months before approving surgery.

If your insurance requires it, the 3-month version is structured very differently from the 2-week protocol:

3-month structure

Months 1-2
These months are gradual lifestyle changes, such as calorie reduction to 1,200-1,500/day increased protein, eliminated sugar-sweetened beverages, daily food logging and monthly check-ins with your physician or dietitian.

Final 2 weeks

The weeks are a transition into the 2-week pre-op liver-shrinking protocol.

The goal of the 3-month version is not weight loss but rather documented compliance for insurance and building habits that will make post-op success more likely.

Realistic weight loss expectation is 5-10% of body weight over 3 months.

For patients on the 1200 calorie pre bariatric surgery diet here’s a day:

1. Breakfast is 1 protein shake blended with 1/2 cup berries.

2. Mid-morning snack is 1 cup cucumber slices with 1 tbsp hummus.

3. Lunch is 4 oz grilled chicken breast, 1 cup steamed broccoli and a large mixed-green salad with 1 tbsp vinaigrette.

4. Afternoon snack is 6 oz Greek yogurt.

5. Dinner is 4 oz baked salmon, 1 cup roasted asparagus and 1/2 cup cauliflower rice.

6. Evening snack is 1 protein shake.

7. Daily totals are 1,200 calories, 95g protein, 40g carbs and 30g fat.

Pre Bariatric Surgery Liquid Diet- What to Expect

The pre bariatric surgery liquid diet is the most challenging phase.

Here’s what’s actually happening in your body:

Days 1-3:

Your body is depleting glycogen, which is stored carbohydrate in your liver and muscles. Each gram of glycogen holds 3-4 grams of water so you’ll lose 4-8 pounds of water weight. Side effects include hunger, headache, fatigue, irritability and dizziness. This is normal and temporary.

Days 4-7:
These days are when your body shifts toward burning fat. Mild ketosis sets in. Hunger drops noticeably. Energy starts returning. Most patients report this is the inflection point where the diet becomes manageable.

Days 8-14:

Many patients describe stable energy and reduced hunger by this point. Bowel movements become less frequent which is normal on a fiber liquid diet.

a. Constipation, which can be relieved by drinking fluids and asking your team about a stool softener.

b. Headaches, which are usually fluid or electrolyte related and can be relieved by increasing water and adding a sugar-free electrolyte drink.

Your doctor will not usually order tests to check if your liver has shrunk. They will check it when they do the operation. If you have followed the rules of your diet your liver will be smaller and softer than it would be if you had not followed the rules.

Bariatric Surgery Pre Op Diet Cheating- What Actually Happens

This section is what every patient looks for. Most clinics don’t talk about it openly. Bariatric surgery pre op diet cheating has consequences and its good to know about them.

If you have one small slip (a piece of bread, a cookie):

Get back on the diet right away. If it happens in the week it probably won’t affect your liver size much if you stick to the rest of the diet. Don’t be too hard on yourself. Just get back on track.

If you cheat repeatedly or in the final week:

This is more serious. Your liver shrinks the most in the week. If you cheat a lot in the 7 days your liver might not shrink as much as it should. Your surgeon might have to do an operation with a longer recovery time.

Liver Shrinking Diet Pre Bariatric Surgery- How to Tell It’s Working

Most patients can tell if the liver shrinking diet pre bariatric surgery is working without tests. Here are the signs:

1. You lose 5-15 pounds, in 2 weeks (most is water., you lose fat too)

2. You feel less bloated and uncomfortable

3. Your clothes fit looser around your waist

4. If you’re diabetic your fasting blood sugar improves

5. If you have blood pressure your readings go down

Your surgeon won’t usually do tests to check if your liver is shrinking. They’ll check during surgery. If you follow the diet honestly your liver will look smaller and softer.

Pre-Op Diet by Procedure Type

The diet you need to follow before you have surgery is pretty much the same for all types of surgery but there are some small differences:

A) Pre-Op Diet for Gastric bypass: you need to follow a special diet for two weeks. The day before your surgery you need to take some medicine to clean out your bowels. For the 24 hours you can only have liquids.

B) Pre-Op Diet for Gastric sleeve: you need to follow a diet for two weeks. You do not usually need to take any medicine to clean out your bowels. For the 24 hours you can only have clear liquids.

C) Pre-Op Diet for Duodenal switch and SADI surgery: you often need to follow a diet for three weeks because people who have this surgery usually have a higher body mass index. You need to take some medicine to clean out your bowels. You need to be very careful about what you eat so that your liver will shrink.

D) Pre-Op Diet for Bariatric Revision Surgery Texas: the diet you need to follow will depend on the type of surgery you had before and the type of surgery you are going to have. You often need to follow a diet for three or four weeks. Your doctor will give you a plan to follow.

What to Do the Day Before Surgery

4. The morning of your surgery you need to drink one bottle of the special carbohydrate drink on your way to the hospital. You need to stop drinking all liquids at the time your doctor tells you to.

5. After midnight you cannot have anything to eat or drink. Not water or gum. Unless your doctor says it is okay.

Remember to bring a water bottle with you to the hospital so you can drink water after your surgery. Wear clothes and do not wear nail polish, makeup or contact lenses.
