The Duodenal Switch preserves the pyloric valve (preventing dumping syndrome), creates a longer intestinal bypass for greater malabsorption, and typically achieves higher excess weight loss (70-80% vs 60-70%). It also has a superior type 2 diabetes resolution rate (>95%). However, it requires stricter lifelong vitamin supplementation.

You may be a candidate if you have a BMI of 50 or higher, or a BMI of 40-49.9 with severe obesity-related conditions such as type 2 diabetes, sleep apnea, or heart disease. The DS is often recommended for patients with the most severe obesity or those who need maximum metabolic benefit.

Risks include bleeding, infection, blood clots, and anesthesia complications. Specific risks include protein malnutrition, fat-soluble vitamin deficiencies (A, D, E, K), loose stools, and gas. Most patients stay 2-4 days in the hospital and return to work within 2-4 weeks depending on their job. Full recovery takes about 4-6 weeks. The DS is a more complex surgery than the sleeve or bypass, so recovery may take slightly longer.

Lifelong supplementation is critical and includes a bariatric multivitamin, calcium citrate with vitamin D, iron, vitamin B12, fat-soluble vitamins (A, D, E, K), and zinc. Regular blood work every 3-6 months in the first year and annually thereafter is essential to monitor nutritional status.

While technically portions of the DS can be reversed, it is considered a permanent procedure. The sleeve gastrectomy component removes part of the stomach permanently. The intestinal bypass portion could theoretically be reversed, but this is extremely rare and not recommended.

You’ll progress through dietary stages similar to other bariatric procedures: clear liquids, full liquids, pureed foods, soft foods, then regular foods over 6-8 weeks. Long-term, you’ll focus on high-protein meals (80-100g protein daily), limit fat and sugar, and eat small frequent meals. Fat malabsorption means high-fat foods may cause digestive issues.
